- Marble plaque in two joining fragments, missing the upper right corner (which was however still preserved when seen in the late 19th century by Kaibel and Mommsen). Part fo the surface is missing lower right (seemingly cut away in antiquity). The rear is finished smooth, but with a narrow margin roughly cut back along the rear of the left edge from the lower corner to c.3 cm from the top.

Inscription
- Layout
- Six lines, of which lines 2-6 are indented by one letter compared to line 1, with a consistent left margin. The final line is shorter with the numeral larger and roughly centred. Mommsen suggested that the age formula in lines 5-6 was a later addition, seemingly for its slightly greater irregularity.
